Opening of New Delhi TB Centre by Her Excellency, the Marchioness of Linlithgow on 20th November 1940 was one of the most important events in the history of Tuberculosis control in India.  New Delhi Tuberculosis Centre was established in 1940 as model clinic in pre independence era by an agreement between TB Association of India (which is registered society under Societies Registration Act no. XXI of 1860 having registration no. 143 of 1938-39) and then Government of India.  The aim for establishing NDTB Centre was to provide; a) Diagnosis and treatment of TB, b) Domiciliary service (TB Care) and c) Training and Research. NDTBC established as a Model TB Clinic way back in 1940, has now become a referral centre for patients of TB and respiratory diseases. The Centre now also functions as Delhi State TB Training and Demonstration Centre (STDC) as well as Intermediate Reference Laboratory (IRL) for Delhi state and is also NABL accredited. As a referral centre, NDTBC provides consultation and diagnostic services to patients referred from private practitioners and TB centres at Delhi and neighbouring states. As STDC, the Centre provides induction and refresher trainings for medical and para medical staff working under NTEP. Trainings imparted are modular, virtual as well as hand-on in laboratory techniques. In addition, faculty of NDTB Centre is involved in monitoring of TB Control activities and analysis of programme data periodically. As IRL, laboratory of Centre is providing diagnostic and follow up services to drug sensitive and drug resistant cases. Our laboratory is fully equipped with all rapid diagnostics via liquid culture (MGIT) molecular diagnostics (PCR-LPA, CBNAAT, ultragene expert) and latest addition being gene sequencer. Microbiologist and technical officers are recognised trainers for national level laboratory techniques. During recent times of COVID pandemic, the laboratory of the Centre has started COVID 19 diagnostic services with help of existing CBNAAT and TreuNAT machines.  In addition, microbiologists of our laboratory are providing training to microbiologists and laboratory technicians of both public and private sector laboratories.  Faculty of the Centre are also in team of Delhi state supervising and monitoring ICMR approved COVID laboratories in Delhi in COVID testing. The Centre is actively involved in teaching of medical and para medical students. The Centre runs a NTEP accredited three months TB Supervisory course round the year. The pass-outs are considered for the post of DOT Provider and TB Supervisors. The candidates from various states join the Course. In addition, MBBS students and interns of MAMC and post graduate students (DNB) from RBIPMT are regularly posted here. Research is also an important activity of the Centre. The faculty participates in various state level operational research and national level research studies.  In recent years, Active Case Finding has been conducted as operational studies in vulnerable populations like prisoners, female sex workers, night shelter inmates, intravenous drug users and homeless pavement dwellers. Latest activity started is ECHO Platform based clinic. The Centre conducts Clinic for Para medical staff on first Wednesday of month on any current subject. As per recommendations of PMDT guidelines 2021, this session is now named as State D3T (State Difficult to Treat TB Clinic) since August 2021.  All District TB Officers, Medical Officers and Private Practitioners in the state can send their difficult to manage drug resistance cases for discussion in the SD3T Clinic.  Faculty from state academic institutes and leading private practitioners are chosen expert for the session.  The cases are referred to this clinic in standard pre-designed format.  There is also a plan to start this clinic for medical college faculty and major private hospitals of Delhi. In COVID era, all programme review meetings and monitoring of microscopic centres are also being conducted on this platform in addition to CMEs and task force meetings. We are grateful to TB Association of India for guiding us in managing the activities of the Centre and to Ministry of Health & Family Welfare for providing us regular grant for running the Centre. I will like to place on record excellent services by the dedicated staff of NDTB Centre for constantly enduring to take up newer activities as per need of the hour.
